11 Questions to Ask in a Workshop Registration Form

A workshop registration form needs 7 to 11 questions: contact details, the session, the details you need to prepare, such as level or dietary needs, and agreement to your terms. Everything else can wait for the confirmation email.

Why the right questions matter

Registrations collected by message end up in three places: the chat, a notebook and your memory. On the day, someone is missing from the list, nobody knew about the gluten allergy, and two people were promised the last place. A registration form gives you one list, with the same details from everyone.

It also helps you prepare. Knowing the level of the group, how many people chose each date and what they need to eat lets you buy the right materials and plan the right pace. The questions below fit workshops, classes, courses and events, and each one says why it is there and how to word it.

Field type: Long text · Optional

9Choice of materials or options

Why ask it: If participants choose a kit, a size or a menu, ask now so you can order the right amounts.

What not to ask

  • Their full address. Unless you post materials, you do not need it. It makes the form longer and asks for data you will not use.
  • Date of birth for adult workshops. If age does not matter for the activity, do not ask. For children's programmes, the grade or age group is usually enough.
  • Payment card details. Never collect card numbers in a form. Put payment instructions in the description or the confirmation instead.

How many questions should a registration form have?

Between 7 and 11. Ask what you need to prepare for the day, and send everything else in the confirmation.

How do I stop registrations when the workshop is full?
