10 Questions to Ask in an Appointment Booking Form
An appointment booking form needs 6 to 10 questions: name, phone, the service, a preferred date and time, and only the details that change how long the appointment takes or how you prepare for it. End with your cancellation policy, so clients agree to it before they come.

Why the right questions matter
Booking by private message usually takes four or five messages: which service, which day, which hour, is this the first time. A booking form asks all of them at once, so the first message you send can already confirm the appointment.
The right questions also protect your day. A service that takes longer than you planned, an allergy you heard about too late or a client who did not know your policy can each cost you an hour. The questions below work for salons, beauticians, nail technicians, tutors, photographers and garages, and each one shows how to word it.

Client details
1Full name
Why ask it: So you know who is coming and can find the client again in your records.
Example wording: What is your full name?
Field type: Short text · Required
2Phone number
Why ask it: The phone is how you confirm the time, send a reminder or move the appointment. For most small businesses it matters more than email.
Example wording: What is your phone number?
Field type: Phone · Required
The appointment
3Service
Why ask it: The service decides how long to block and what to prepare. List your real services, and add a price or duration next to each if you want fewer questions.
Example wording: Which service would you like to book?
Field type: Dropdown · Required
4Preferred date
Why ask it: A preferred date lets you check your diary and suggest the nearest free slot.
Example wording: Which date suits you best?
Field type: Date · Required
5Preferred time
Why ask it: Some clients can only come early or late. Ask for a time, or for a part of the day if your slots are flexible.
Example wording: What time suits you best?
Field type: Time · Required
6First visit
Why ask it: New clients often need a longer first appointment for a consultation, a patch test or paperwork.
Example wording: Is this your first visit with us?
Field type: Single choice · Optional
7Preferred staff member
Why ask it: If you have a team, regular clients want to stay with the person they trust. Add "no preference" to fill gaps in the diary.
Example wording: Who would you like to book with?
Field type: Dropdown · Optional
Before you arrive
8Details that change the length
Why ask it: Every business has one: removing an old nail set, hair length, the car model, the number of people for makeup. Ask it so the slot you book is long enough.
Example wording: Do you need an old set removed? / What is the make and model of your car?
Field type: Single choice · Optional
9Health notes or allergies
Why ask it: For treatments that touch skin, hair or nails, allergies and sensitivities must be known in advance. Keep the question optional and clearly worded.
Example wording: Do you have any allergies or sensitivities we should know about?
Field type: Long text · Optional
10Agreement to your policy
Why ask it: A required checkbox with your cancellation or late policy means every client has read it before the appointment, which makes no-shows easier to handle.
Example wording: I have read and accept the cancellation policy.
Field type: Agreement checkbox · Required
What not to ask
- Their full address, unless you go to them. For appointments at your place, an address is one more field with no use. Mobile services are the exception.
- Email and phone both as required. Most clients answer messages faster than email. Ask for one as required and the other as optional.
- Medical history in detail. Ask only what affects the treatment. Long health questionnaires make people stop halfway.

Frequently asked questions
How many questions should a booking form have?
Between 6 and 10. Clients book from their phones, so keep it to what changes the length of the appointment or how you prepare.
Does the form show which times are free?
Not in Pinkit. The form collects a preferred date and time, and you confirm the appointment with the client.
Should I show prices in the booking form?
It helps. Prices next to each service answer the most common question before it is asked.
Can clients choose more than one service?
Yes. Use checkboxes instead of a dropdown for the service question.
How do clients find the booking form?
Share the link in your Instagram bio, your Google Maps listing, your website and in replies to messages that ask for an appointment.
