How to Turn a Word or PDF Document Into an Online Form

To turn a document into an online form, attach the file to an AI form builder that reads documents. In Pinkit Forms you attach a PDF, a Word (.docx) or text file, or a photo of a printed form; the AI copies the questions with their original wording and order and picks a fitting type for each one (short answer, multiple choice, date, phone). You then check it, change anything in the chat, and publish it as a link.

Step by step

  1. Open the chat. Go to pinkitforms.com.
  2. Attach the file. Use the attach button. Choose a PDF, .docx or .txt file, or a photo (PNG, JPG, WebP) of a paper form. Files up to 5 MB work.
  3. Add a sentence if you want. Without a message, the AI copies the questions as they are. You can add instructions: "copy the questions and make all of them required" or "leave out the office-use section".
  4. Check the result. Compare the form with the document. Answer choices that were checkboxes on paper become checkboxes; lines for writing become text fields.
  5. Publish. Preview it, publish it and share the link. Answers arrive in your account instead of on paper.

What kind of documents work

Anything that already has questions: intake questionnaires, registration forms, health declarations, order sheets, feedback forms, exams and worksheets. For printed forms, a clear photo taken straight from above works best.

If the document has no questions, for example an article, a syllabus or a price list, the AI writes questions based on it instead: a quiz about the article, or an order form from the price list.

Tips for a clean result

  • One form per file works best.
  • Crop photos to the form and avoid shadows.
  • If the document is long, check the end of the form; ask "did you copy all the questions?" if something is missing.
  • Fields such as signatures and file uploads do not exist in Pinkit Forms yet; the AI replaces them with the closest question type and tells you.
